Help with Getting Started on the Annual Hazardous Waste Report
- What if I want to use hardcopy paper forms to report?
- Call the Annual Report Hot Line in Portland at 503-229-6938 or
toll-free in the State of Oregon at 1-800-452-4011 x6938.
- Send a request by e-mail to
hazwaste@deq.state.or.us.
- Download and print the Site ID Form and the Waste Generation and
Management Waste Stream Form from HazWaste.net. Instructions
for downloading are provided in questions 12 and 13.

- How do I bookmark HazWaste.net?
Go to www.deqhazwaste.net and then bookmark this main
page by adding to your favorites. If you bookmark the HazWaste.net
"Sign-In" page, you will receive a HazWaste.net System Error message
telling you cookies must be enabled.
- How do I get started if this is my first time using the
HazWaste.net electronic annual reporting software?
New users to electronic reporting will start at
Creating a User Profile. Previous electronic reporters will use their
username and password and start at the "Sign-In" page. You may find
it useful to print the step-by-step instructions found on the main Web
page or
General Instructions.
If someone from your company previously filed an
annual report electronically and created a user profile, they will be
the Data Administrator and you will have a Data Entry User Role. A Data
Entry User will not have the ability to submit the report to DEQ.
Questions 5 and 6 will further explain this situation.
- What is the difference between a Data Administrator and Data Entry
User?
The annual report reminder letter mailed in December of each year
includes the names of active Data Administrators from previous reporting
years. The Data Administrator is usually the first person to set up a
user profile in HazWaste.net or a subsequent user who has had their user
role updated. A Data Administrator has annual report submission rights.
Unless designated as a Data Administrator, other users only have data
entry roles. Data entry users cannot submit the completed report to DEQ.
Once you sign in to HazWaste.net, your user name and role is displayed
on the top left corner of the screen. If you need your user role
changed, follow the instructions in the question 6.
- How do I get my user role changed?
- Ask the previous Data Administrator to change your user role by
clicking the "Admin" tab located at the top of the Reporting Forms Log
screen and click the "Edit Role" link to update roles; or
- Call or e-mail the Annual Report Hotline and ask that your user role be
changed to Data Administrator. In Portland call 503-229-6938 or
toll-free in Oregon at 1-800-452-4011 x6938. An e-mail request
can be sent to hazwaste@deq.state.or.us.
- If my company previously reported electronically, how do I get
started with my User Name and Password?
To get to the HazWaste.net Web page, type
www.deqhazwaste.net in your internet browser. From this main page you can either scroll down
the page to review and print the step-by-step general instructions or
just click on the link to open HazWaste.net.
If you have a username and password:
- Enter your user name and password; this will take you to the Assigned
Sites page. Click on your RCRA Site ID number to open up the Reporting
Forms Log page. Under Annual Report Form History, create a Site ID Form
and then enter your waste generation and management information on the
GM Forms.
If you forgot your username or password:
- For the user name, refer to the annual report reminder letter mailed in
December of every year or call the Annual Report Hotline in Portland at
503-229-6938 or toll-free in Oregon at 1-800-452-4011 and ask for
extension 6938.
- For your password, follow the instructions in the question 8.
- I forgot my password, what do I do?
- Go to www.deqhazwaste.net
- Click on the HazWaste.net link to get to the "Sign-In" page.
- Click the "Request Password" link in the grey box located to the right of
the "Sign In" window.
- Type your e-mail address. Use the same e-mail address you entered when you
created your user profile.
- You will receive an e-mail from the HazWaste.net system notifying you of
the new password. The new computer generated password will be all
numbers. You can change this password by following the instructions in
the question 9.
- How do I change my password?
- Go to www.deqhazwaste.net
- Click on the HazWaste.net link to get to the "Sign-In" page.
- Sign in using your user name and password; click the "Submit" button.
- Locate the two tabs "Home" and "Profile" located at the top right side of
this page.
- Click the "Profile" tab, and click the "Change Password" link located in the
grey "Change Password" box.
- Follow the instructions to create the new password; click the "Submit"
button.
- Click the RCRA Site ID number to open the Reporting Forms Log page.
- What if a change of ownership occurred?
A new owner or their representative must submit the Site ID Form
by mail since a signature is required. You can print a pre-populated SI
Form in HazWaste.net by following the instructions in question 12. The
pre-populated SI Form has the last reported contact information with
space to provide updated information. You can also call the Annual
Report Hot Line in Portland at 503-229-6938 to request the forms by
mail.
Once the SI Form is received by DEQ from the new owner, the waste
generation and management information can be submitted electronically.
You can also choose to submit hardcopy GM forms with the change in
ownership SI Form. To print the GM Form, follow the instructions in
question 13.
- Do I need to file a report if the business is no longer operating?
Yes. You need to file a Site Identification Form either
electronically or by mail to withdraw the RCRA Site ID number. You must
verify the generator status for the part of the year you were in
business. If your business was a small or large quantity generator you
must also file the Waste Generation and Management (GM) Forms.
- How do I print a Site ID Form from the HazWaste.net system?
The Site ID Form printed from HazWaste.net will include the
previously reported information.
- New electronic reporters need to create a user profile in HazWaste.net.
Instructions for creating a User Profile can be found in the
General Instructions.
Once you have created a username and password follow these steps:
- Sign-In to HazWaste.net at www.deqhazwaste.net using your username and
password.
- Click on the RCRA Site ID number on the Assigned Sites page. This takes
you to the Reporting Forms Log page. At the top of the Reporting Forms
Log page, click the "Open Pre-Printed Site ID Form" link.
- Select
"File" and "Print."
- Where do I find the GM Form and the associated Code lists?
Go to www.deqhazwaste.net and scroll down towards the bottom of the
HazWaste.net main Web page. Open up the links to print these documents
to report your waste stream information on hardcopy forms.
- What if I need an extension to the reporting deadline?
If you need an extension to the deadline, please e-mail your request
including your RCRA Site ID number, before the March 1st deadline to
hazwaste@deq.state.or.us. You can also fax a request to the Hazardous
Waste Forms Clerk at 503-229-6977. You will receive a confirmation
e-mail or fax extending your due date to April 1st.
